Description
| Description (Service) | Provides supports and services to families with children 0-18 years of age with a developmental and/or physical disability who are living at home. Facilitators support families to develop a plan of care, complete relevant applications, provide support for advocacy, make referrals, explore resources and natural supports, and support transition planning to Adult Services. |
