| Application | Current patients: call to book appointment. For those who do not have a health care provider (physician or nurse practitioner) or if the health care provider is more than 50 km from the applicant's home, applicants can fill out the application form below and send by mail or email to office (address on form). Patient Application All applicants must have proof of a primary address in the Trent Hills service catchment area and a valid Ontario Health Card. Priority is given to those who live in Trent Hills.
