Description (Service) | Central Intake offered by Community Support Service Providers in Huron and Perth. Services include information about existing services, accessing new services, changing or cancelling current services. Meals and Nutrition - Community dining, Meals on Wheels (hot and frozen), and nutrition screening Education and support - adult day program, individual and group supports, caregiver supports, education, information and system navigation Safety and Reassurance - Visiting and telephone reassurance, education and behavioural supports, support on hospital discharge, and personal emergency response systems Health and Wellness - Social programs, group fitness, self-management, bathing, health education, falls prevention, and medication management supports Support in the Home - Personal supports, house keeping, in-home meal preparation, in-home exercise, respite, and caregiver relief Transportation - Rides to medical appointments, social activities, errands, and shopping. Escort services available. Includes rides to vaccination clinics Intensive Support Programs - Assisted living, adult day programs, overnight and day respite, and supportive housing Specialized Services - Supports for people with memory loss/dementia, acquired brain injury, adults with physical disability * attendant outreach, counselling, and social work supports Participating agencies include: Alzheimer Society of Huron County Alzheimer Society of Perth County West Huron Care Centre - Community Connections Outreach Services Cheshire Dale Brain Injury Services Family Services Perth-Huron Mitchell and area Community Outreach Services ONE CARE Home and Community Support Services Community Outreach Services - Milverton Spruce Lodge Continuum of Care St. Marys and Area Friendship Centre St. Marys and Area Home Support Services St. Marys and Area Mobility Service Victorian Order of Nurses - Perth-Huron Branch |
